A Discharge Planner's Home Safety Checklist for Long Island Patients

September 29, 2026

Discharge planners, social workers, and admissions coordinators handle dozens of transitions a month, each with its own home safety picture. Most of the patients behind those transitions are seniors, and most of the decisions about their home get made in a phone call with an adult child who isn't in the building and may not be local at all.

We have written an easy-to-understand checklist you can walk through with a patient or family, or hand off directly, when the bathroom comes up in discharge planning.

What To Ask The Patient Or Family

  • Can they currently step over their tub wall safely, given their mobility aid and any weight-bearing restrictions from a hip, knee, or back procedure?
  • Is there a shower chair, raised toilet seat, and grab bars already in place at home, or do these need to be sourced before discharge?
  • Is the home layout wide enough for a walker or wheelchair to move through the bathroom doorway?
  • What is the realistic discharge date, and has the family started any home preparation yet?
  • Will the patient be returning to their own home, or moving in with an adult child, and does that change what needs to be assessed?

When To Flag A Bathroom Modification As Likely Necessary

A modification beyond grab bars is usually the right conversation when a patient has a weight-bearing restriction that makes stepping over a tub wall unsafe, when a fall risk assessment specifically identifies the bathroom, when the home has only a standard tub with no existing accessible shower option, or when the patient will be living alone with limited daily supervision after discharge.

What To Tell Families About Timeline And Cost

Two things families consistently get wrong on their own: they assume Medicare will cover a structural bathroom modification, which according to Medicare.gov's own coverage guidance it does not, and they assume a fix will take as long as a full renovation. Both assumptions add stress at exactly the wrong moment, often surfacing as anxiety directed at facility staff who aren't the ones who can solve it. A step-in shower conversion is a middle option worth mentioning by name: it addresses the tub wall specifically, starts at $1,795, and is installed in under three hours, which fits inside most discharge timelines without requiring a renovation-scale project.

Talking To The Adult Child, Not Just The Patient

In practice, the person you're coordinating with is often an adult child managing this by phone, sometimes from out of state, often juggling their own job and family alongside a parent's discharge.

A few things tend to reduce their anxiety in that conversation: telling them a bathroom assessment doesn't require anyone to be physically present, since phone and photo assessments can happen while the patient is still admitted.

Being specific about cost ranges rather than vague reassurance, since sticker shock after discharge is one of the most common sources of frustration directed back at facilities; and being clear about what is and isn't covered by insurance, so the family isn't blindsided later.

How A Contractor Assessment Can Happen Before Discharge

Affordable Bathtub Conversions does phone and photo assessments while a patient is still in your facility, so there's no need to wait for discharge to get a plan in place. We provide a licensed, itemized estimate within a day or two at no charge, whether or not the family ultimately moves forward. If a veteran's case involves the VA HISA grant, we prepare estimates in the format the application requires.

Working Directly With Your Facility

If your facility regularly refers patients for home safety modifications, we're glad to be a resource you can point families to directly, or a call away if your team wants to talk through a specific case before discharge. We serve facilities and families across Nassau and Suffolk County, and there's never an obligation attached to a conversation.

Frequently Asked Questions

Can you assess a bathroom before the patient is discharged?

Yes, using phone or photo assessment. This is the most common way we work with facility referrals, and it works even when the adult child managing the decision isn't local to the home.

Do you charge for the estimate?

No. The itemized estimate is free whether or not the family decides to move forward.

How fast can installation actually happen once a family decides to proceed?

Typically within days of the initial call, with the installation itself completed in under three hours.

Do you work with veterans applying for the VA HISA grant?

Yes. We provide HISA-formatted contractor estimates and can walk families through the general process, though we don't process the VA application itself.

What should I tell a family who assumes Medicare will cover this?

It's worth correcting early. Medicare.gov's durable medical equipment coverage page does not include structural home modifications, so families should plan around a private-pay option rather than expect reimbursement, unless a Medicare Advantage plan offers a specific home safety allowance.

What if the adult child managing this lives out of state?

That's common, and it doesn't slow things down. We can coordinate an assessment using photos taken by a family member, home health aide, or facility staff, and communicate estimates and scheduling entirely by phone or email.
